If you are committed enough to change your life, I have payment options available for you to accommodate your financial needs.
Health-savings (HSA) accounts, Flexible Spending (FSA) accounts, “Benny”, Debit and Credit cards are welcome forms of payment.
I do not accept: Insurance, personal checks, business or corporate cards, Discover or American Express cards.
Payment to Hold Appointment:
All appointment sessions must be held with full office visit fee deposit.
This deposit will be credited towards office visit fee upon arrival of appointment, or refunded in full when given more than 48 hours courtesy notice for appointment changes.
Cancellation Policy:
In respect of my extensive priority list, I require a minimum 48 hour notice (business days only) for all appointment changes and cancellations.
If the 48 hour cancellation courtesy is not provided, 50% of the visit fee will be applied towards the non-refundable cancellation charge.
Future visits will not be authorized if there are late cancellations or unkept appointments.
